Dr. Haleh Nekoorad-Long, M.D., is a board certified geriatric psychiatrist and the full-time medical director at AltaVita Assisted Living Memory Care Centre. She has extensive experience caring for aging adults facing dementia and its challenges. Dr. Nekoorad-Long has operated the Longmont-based Colorado Mood and Memory Clinic since 2003. She is on-site at AltaVita Assisted Living Memory Care Centre and ensures we are using the latest strategies to implement individualized care. We have hand-picked some of the most talented individuals to be part of our team, with the goal of keeping our residents stimulated physically and mentally.

We pride ourselves on providing the most modern, high-quality amenities, services and activities designed to enrich the lives of our residents. We ensure our services and programs are in line with proven Alzheimer’s disease, dementia and other memory-related illness treatments. Our care plans include enhanced assisted living for the more independent and high functioning resident to inviting hospice in when appropriate . From dining to daily programs, housekeeping to health we provide meaningful and individualized amenities, services and activities that allow each resident to live a purposeful and enjoyable life while receiving the dignified and loving support they deserve.
AltaVita Assisted Living Memory Care Centre features four distinctly themed but interconnected neighborhoods that allow for ample space to stroll or explore unique points of interest. A spacious interior courtyard rests at the heart of the community. This welcomes residents to enjoy the outdoors all year round while also providing warm, soothing natural light throughout the halls and common rooms.
Caring for an individual with memory care issues at home can be challenging. Our adult day care program provides short-term relief for hardworking caregivers and offers individuals with memory related illnesses a new, engaging environment to visit several times a week. Our respite care program offers overnight stays for guests with memory-related illnesses so caregivers can enjoy a reprieve from the rigors of daily care giving. Program participants receive the same comprehensive care as our permanent residents. This provides peace of mind for caregivers, family members and loved ones.
