Each resident will have input in a customized care plan under the direction of their physician with the guidance of Hillhaven’s Medical Director (Board certified in Gerontology and Internal Medicine). The Care Plan Team includes members from: nursing, nutritional services, social services, rehabilitation and activities. Residents and their families are invited to attend and be part of the Care Plan Meetings.
