HomePlace Memory Care at Oak Harbor
Home Place Oak Harbor is a unique home-oriented Alzheimer’s and Dementia Care Facility. We specialize in providing the highest quality of care to our resident’s, by employing caregivers and nurses who are kind, compassionate and educated to this level of care.
We have 3 living areas with a total capacity for 50 residents.
Meadows has a capacity for 18 residents, and it is for those needing the highest level of care, and occasionally, it is for residents who need a quiet and less stimulating environment. Most of these residents require total assistance with their activities of daily living.
Woodlands has a capacity for 16 residents, who are mostly independent with walking, and with much of their personal care. They need a secure environment to ensure they do not wander away.
Seaside has a capacity for 16 residents. These folks are in-between independent and heavy care. They are usually very busy, and may need our special dementia redirection, or distraction, skills.
All of our communities have a centralized area within for activities and special events. Each has lots of large windows that let the sunshine in, and each community opens onto our patio, which is secure, and planted with beautiful and safe plants. We pipe music onto our patio, and the pathways allow our residents to wander from community to community, giving them a real sense of freedom.
Each community has its own personality, yet all look very similar, allowing our resident to transition to different levels with ease. Each community has its own activities, geared to the level of folks who live there. We believe in activities that ‘replace lost needs’and that bring residents moments of joy. We think any activity can be adapted to work for the level of folks it is serving. Hence, our activity program is adventurous, and it is full.
Our goal everyday is to provide the highest level of care, and to treat each resident with the utmost respect and dignity, while providing a home-like environment.

Family participation is strongly encouraged to help maintain open lines of communication via phone, e-mail or fax– to ensure all care concerns are managed and addressed as the needs arise. ‘Our home is your home’, and we encourage our families to visit wherever they can.
