Aging Gracefully Family Home

Aging Gracefully Family Home is owned and operated by Registered Nurses.

Dekyi Yangzom R.N is a critical care nurse, currently working as an open heart nurse in a Cardio Vascular Intensive Care Unit at Peace Health hospital. Dekyi has over 20 years’ experience working with adult and elderly population.

Sonam Wangchuk R.N. has worked as registered nurse in Hospital, nursing home and rehab unit. Currently working full time as resident care manager at Aging Gracefully Family Home.

We both have the same passion and experience to provide a compassionate care. We are committed to providing quality care; maintaining the health, independence, dignity, and quality of life in our residents’ retirement years.

Our home is located in a quiet residential neighborhood, in Vancouver, Washington- close to Peace Health Hospital and easy access to I-205 & I-5.

We offer a place where residents live in a family environment, where they are cared by registered nurse and certified nurse aids 24/7.

Unlike most large retirement and assisted living facilities who typically average 15 residents to every 1 caregiver, we pride ourselves on a caregiver-to-resident ratio that is unsurpassed in the industry: Aging Gracefully provides 1 to 2 caregiver to 4 to 6 residents in our home.

Our staffs are experienced, board certified, and has received training in Dementia and Mental disorder approved by DSHS.

Food is home-cooked, nutritious and tailored to resident’s tastes and needs.

Our entire home is wheelchair accessible.
