WELCOME to Golden Swan Assisted Living Facility, (6 beds)
.’A Place like Home’.
We are a family owned, with several years of experience in senior care. We specializes in caring Patients with dementia or Alzheimer’s disease.
We cater 6 seniors 24 hrs. per day, by 2 professional CNA/HHA providing in-home care Assistance with full Daily activities.(Medications, Bathing , Toileting, Dressing , grooming , eating, Transferring, etc.
Also Doctor/ARPN visits , Specialists ( Neurologist , Physiologist, Physiatrist, Dental & Oculist) RN & LPN nurses as needed, Lab, X-rays, EKG’s, therapies, wound care and more services in the facility without any extra cost.
We accept : VA, Medicare, Medicaid, , Long Term Insurance & Private funds. Prices are affordable & competitive. We can assist you to apply on State and Veterans Programs to pay
perhaps in full our place .
Available Respite care ‘Enjoy your vacation time, business or something that you need to do’… You can feel Free and with absolute peace on mint , leaving your loved one with us for a specified period to attend to your own needs. (Rates are available per day / week)
We also Offer Hospice Care to our Permanent Residents.
Everyone here is considered a family member. We offer you as much care, independence, privacy, dignity and companionship as you want .

Golden Swan is a luxury Safety Home, ‘It’s a Private House, no an Institution’ with exquisite decoration, beautiful gardens, comfortable accommodations , full alarm system(Fire and doors)We only provide 6 gorgeous Private Rooms (furnishing if you want & services are included) Our facility is located in ‘Tall Trees’, Coconut Creek, across street on North Broward Preparatory school by Lyons Rd. Between Hillsboro Rd. and 18 street (just in the middle of Coconut Creek & Boca Raton)
Golden Swan offers you three daily healthy and delicious meals ,desserts, alternates ,are ‘home made’ .
Our menu is by a Certified dietitian and nutritionist. Snacks, fresh fruits and drinks all day.
Daily activities for mint & spirit (In & Out doors) Doctor transportation, outings, beautician, Pool Safety, Birthday and holidays celebrations, brunch and live music every Saturday and more …
